COR Program Application

This form is used if you are able to continue to house the pet in need of finding a new family. The pet must be current, or will be brought current soon, on necessary medical to participate.

Participation in this program does not surrender the animal to Heartland Husky Rescue in any way. In the event we assist in placing the pet, the adopting party will be signing a Heartland Husky Rescue Adoption Contract. You will then be surrendering the pet at that time.

If we have your pet listed with us, please advise us if you find a home on your own for the animal so we can keep our posting current.
